Stand-Alone Medically-Assisted Substance Use Disorder Treatment

$200 for 28 days of care
Includes intake and complete history & physical, general medical services, as well as all services listed above. (Payment may also be made bi-monthly.)

We also offer a separate Concierge Healthcare Program that is a non-insurance direct-pay service. Instead of billing your health insurance, we provide care through a simple monthly membership fee.

Medication-Assisted Treatment Payment and Cancellation Policies

To maintain client privacy, we do not directly bill insurance companies. Your doctor will not discuss your care with insurance providers and will not provide insurance companies, or any third-party payers, access to your diagnosis or treatment records.

However, many clients with private insurance do choose to have their insurance partially reimburse them for our services. Should you request this, we will provide you with a billing invoice that you may submit to your insurance provider, who may reimburse you for our services at your policy’s out-of-network rate.

Medicare and Medicaid? Unfortunately, we cannot currently provide care for Medicare or Medicaid clients.

Intake Paperwork:  For your first visit, please arrive thirty minutes early.

Can’t Make Your Appointment? If you are unable to make your appointment it is important that you call and let us know. Patients 15 minutes late or more for an appointment may be rescheduled for another time.
